Neck wrinkles can have various causes. In general, the body breaks down collagen and elastin over time, which leads to a loss of natural elasticity and a perceived excess of skin, which then appears in the form of neck wrinkles.
With increasing age, the platysma (muscle below the chin) can also become slack. Possible consequences are vertical wrinkles on the neck, sagging skin areas (turkey neck) and a softening of the chin line.
The predisposition to wrinkles in the neck area is largely genetic or anatomical. However, skin care (especially UV protection), sleeping position, diet and lifestyle in general can also have a negative impact on premature skin ageing.
From around the age of 50, many people experience a clearly visible loss of shape or a lack of demarcation between the face and neck. If the neck and lower chin area have excess fatty tissue, severe wrinkles can also form in the neck region. There is also often talk of the so-called “turkey neck”.
A facelift or neck lift provides a remedy by restoring the harmonious overall appearance of the face and neck and improving the definition of the lower jawline.
The SMAS technique is generally used in our clinic. This method not only tightens the skin, but also lifts the underlying tissue layer (SMAS), which is crucial for natural proportions and a clearly defined chin and jawline. However, we have significantly developed this technique further by also applying fixation sutures of different depths under the SMAS platysma layer. This has some decisive advantages:

We also use the very gentle laser lipolysis procedure for patients whose neck does not actually have a pronounced loss of elasticity, but whose subcutaneous fat layer is too thick. Under local anesthesia, the excess fat is first suctioned out through two or three small incisions that will later be barely visible, and then the tissue is tightened with a special laser probe to achieve the perfect neck contour.
However, it is also possible that from birth a certain hardening of the connective tissue (fibrosis) fixes the neck skin to the base in several lines running across the neck (transverse folds) and causes a regular indentation of the skin surface. A neck lift reduces wrinkles somewhat but does not eliminate them. However, wrinkles can be reduced by a combination of various minimally invasive or non-invasive methods independently of a lift.
First, a low-cross-linked hyaluronic acid is injected intradermally into the wrinkles. An erbium glass laser is then used to stimulate the connective tissue in the wrinkle area, which stimulates collagen production. In addition, radio frequency needling treatments can also support wrinkle reduction.
It is also possible that a part of the neck skin (usually in the front area) or the entire neck skin has a wavy and “dry” surface structure. In this case, practically all the elastic fibers of the skin are no longer functional. A facelift can improve the overall appearance of the neck skin somewhat, but has absolutely no effect on the skin quality.
With the non- or minimally invasive treatments mentioned above, a good improvement can be achieved in this case independently of surgery. This is additionally supported by the daily application of a coordinated skin care routine. Proven active ingredients include hyaluronic acid, urea or retinol (in low concentrations) and DEANOL.

Since my beginnings as an independent facial surgeon in 1995, I have been fascinated by the subject of neck lifting and the comprehensive analysis of the aesthetic features of the neck. My fellowship with facelift guru Bruce Connell in Santa Ana, California that same year marked a decisive turning point in my medical career. Under the guidance of Bruce Connell, I was given the challenging task of researching all aspects of neck aesthetics and documenting them in a scientific paper. I took on this challenge with the utmost care and developed the NECK-CHECK, which is still an unsurpassed reference today. The scientific procedure is based on the assumption that there are twenty-two individual criteria for determining the esthetic appearance of the neck. This figure then makes it easier for the surgeon to choose the appropriate corrective measures and methods.
The opportunity to present my analysis procedure at the annual meeting of the American Society of Aesthetic Plastic Surgery in 2005 was undoubtedly a highlight of my professional career and an exceptional recognition of my work.
The NECK-CHECK from back then still forms the basis of my consultations today. The individual risk of the treatment is assessed by taking a medical history. Depending on your state of health, it will be determined which preliminary examinations (e.g. blood test or ECG) are necessary. During the consultation, we will tell you which medication you can take regularly. If there is too great a risk to your health, we will consistently refuse surgery or postpone it until a more favorable time, provided that the risk can be reduced to an acceptable level by means of appropriate therapy.
Thanks to our gentle surgical technique, all lifts can be performed under local anesthesia and twilight sleep. This eliminates the need for stressful anesthesia. Patients then usually spend three nights in the clinic.
In very slim patients with little subcutaneous fat and no disturbing muscle tension in the front of the neck, it may be sufficient to remove excess skin as part of a face-neck lift.
However, if the fat layer under the skin is clearly too thick and the contours in the jaw angle area are not clearly defined, it is necessary to reduce the entire fat layer. In addition, the deep layer (SMAS platysma complex) should be loosened, reduced and tightened separately.
In order to guarantee an optimal quality of the procedure, we never perform an operation under time pressure. Thanks to generous timing and methods that have been tried and tested over many years, our patients have significantly fewer problems with swelling and discoloration than patients who have undergone other methods of face/neck lifting.
Although a facelift can improve the overall appearance of the neck, it is important to understand that it does not necessarily improve skin quality. In such cases, we therefore recommend a combination of surgical and non- or minimally invasive treatments.
Non-surgical options include injections with hyaluronic acid and laser treatment. These procedures can help to improve skin texture and reduce the appearance of wrinkles. In addition, radiofrequency microneedling can also be an effective method of improving skin quality and supporting the results of wrinkle reduction.
We have been working closely with a highly qualified team of anesthesiologists since 1995. During this time, we have continuously worked on making anesthesia for face/neck lifts gentler and safer. This led to the development of a concept for an extremely mild twilight sleep sedation, which is probably unique in aesthetic medicine. A sophisticated combination of sedative medication, a very effective local anesthetic and a relaxed atmosphere in the operating room enables us to perform all lifts without general anesthesia.
This procedure meets the wishes of many patients, as many are more afraid of anesthesia than of the actual procedure. Our concept also offers clear medical advantages. Above all, it significantly minimizes the risk of thromboses and embolisms. In addition, the absence of general anesthesia allows better control of facial expressions during the procedure, which significantly reduces the risk of damage to the facial nerves.
As our patients receive less medication overall, the recovery time after the procedure is also significantly shorter. As a rule, our patients can stroll through our clinic’s park on the evening of the procedure without any problems.
Neck wrinkles occur in the course of the natural aging process and are largely determined by genetic factors and anatomical conditions. Nevertheless, external influences such as UV radiation, diet and lifestyle also play a significant role in their development. It is therefore possible to positively influence skin quality to a certain extent with good skin care, sun protection and a healthy lifestyle.
Exercise can certainly help to strengthen the muscles in the neck area and improve circulation, which can indirectly contribute to skin health. However, a positive effect on neck wrinkles can be considered negligible. Excessive activity of the neck muscles (PLATYSMA) can even promote the formation of wrinkles, which is why the use of botulinum toxin injections can be very helpful in these cases.
Creams and serums can be used to support and above all prevent neck wrinkles. Good active ingredients are, for example, hyaluronic acid, urea or retinol (in low concentrations). In our practice, we have also had good experience with the active ingredient DEANOL – especially in combination with retinol. And last but not least: the use of sunscreen with a high sun protection factor (SPF) is crucial to protect the skin from the harmful effects of UV radiation.
Yes, superficial transverse wrinkles on the neck can be injected with hyaluronic acid and softened to a certain extent. If the muscular neck cords are responsible for the formation of wrinkles, treatment with botulinum toxin can also bring about an improvement.

In contrast, face lifts, in which the tissue between the facial skin and the deeper facial muscles is mobilized and lifted separately, reduce these risks. By integrating the capsule of the parotid gland into the so-called Superficial Musculo Aponeurotic System (SMAS), the preparation of a stable flap is facilitated, which helps to improve the neck contour during displacement and tightening.
The displacement and tightening of the SMAS limit the tightening forces in depth, while the facial skin remains almost tension-free, which significantly reduces the risk of unsightly distortions and noticeable scars.

The classic face/neck lift is a surgical procedure in facial aesthetic plastic surgery in which sagging areas of the face are lifted and sagging areas of skin are tightened. In addition to surgical lifting, there are other procedures such as injections, laser, radio frequency or ultrasound, which also have a tightening effect to a certain extent.
Minimally invasive procedures can be a good option, especially for younger patients or as a complement to facial surgery. In terms of the rejuvenating effect, however, these methods cannot be equated with a surgical lift.
